Cefotaxime Dose

The dose of Cefotaxime in an infant with fever depends on the suspected source of infection, age, and weight of the child.

Neonates (≤28 days)

  • Gestational age <37 weeks (preterm):

    • <7 days old: 50 mg/kg IV/IM every 12 hours
    • ≥7 days old: 50 mg/kg IV/IM every 8 hours
  • Gestational age ≥37 weeks (term):

    • <7 days old: 50 mg/kg IV/IM every 8–12 hours
    • ≥7 days old: 50 mg/kg IV/IM every 6–8 hours

Infants (>1 month)

  • Meningitis: 75–100 mg/kg/day IV divided every 6–8 hours
  • Sepsis/Pneumonia/Severe infections: 100–200 mg/kg/day IV divided every 6–8 hours
  • UTI/Moderate infections: 75–100 mg/kg/day IV/IM divided every 8 hours
